Candidates having the above qualifications should submit their application, educational qualification 12th mark sheet, and all year marks of educational qualification, age document, all Along with self-attested copies of documents and a recent photograph signed on the upper left side of the application "Health Branch Room.No.250 New District Panchayat Bhavan – Morbi So-Ord Shobheshwar Road, Morbi next to Collector Office. Also necessary Compulsory to bring original certificates / supporting documents.Candidate will have to attend the interview at his/her own expense.Final decision on recruitment will be subject to the District Health Society and its terms and conditions.

Note:-(1) Remuneration and other matters of each cadre will vary as per the instructions of the Government from time to time.
