Important Announcement regarding Certificate Verification of Candidates Included in Additional Provisional Merit List 1. Advertisement No. 17/2021-22 Multi Purpose Health Worker (PO) Cadre Competitive Examination was held on 26/06/2022 by Gujarat Panchayat Seva Selection Board, GanŪnagar. The Additional Provisional Merit List of this competitive examination has been published by the Board on 14-02-2013, which has been placed on the Board's website http://apist chart.gov.in.

2. Candidates included in this Additional Provisional Merit Lost are requested to appear in person on the date and time allotted to each candidate at the place mentioned below for verification of original certificate in the following details, the email regarding which is being sent to the email id mentioned by the candidate in his application. Along with the email to be sent to the candidate, a copy of the index of certificates, attestation form (Form-1), and a form showing the details of the candidate's educational qualification (Form-R) will also be sent which the candidate has to read carefully and fill in the necessary details and present at the time of certificate verification.

3. Therefore, all the candidates included in the Additional Provisional Merit List are hereby informed to appear in person at the designated place for verification along with the below-mentioned certificates as applicable (including original certificates) on the date allotted to them. The candidature shall be deemed to be permanently withdrawn and thereafter no claim or dispute in the matter shall be entertained.
