1. Attested xerox copy of last year mark sheet and trial certificate of Bachelor's degree OR Recogrized sanitary inspector course
2. Attested xerox copy of Certificate course in computer operation (inirrurrm 2 month) has to be attached
3. Attested xerox copy of Permanent two wheeler driving license has to be attached.
4. Tuberculosis health visitor's recognized course Govt. Recognized degree/diploma in social work or medical_social work last year mark sheet and attested xerox copy of trial certificate should be attached.
5. Basic training course (Govt. recognized) for Multipurpose health workers last year mark sheet and attested xerox copy of trial certificate should be attached.
6. School Leaving Certificate and Photo I.D. Attested copy of card to be attached.
Merit list will be prepared based on the filled details. If any discrepancy is found in the details filled by the candidates during the face to face verification then the application form of that candidate will be considered as canceled which will be binding on the candidates. Being a contractual appointment, no other vested interest will be available. And the term of appointment will automatically expire on the completion of the contract period. On completion of the contract, a new contract can be made for another 11 months based on performance.
